Elder
Always drops one unique and one Uber Elder fragment (Terror or Emptiness). The fragment covers a good part of the entry by itself.
Watcher's Eye (40%) and Elder's Exalted Orb (12%) do most of the remaining work. Void of the Elements at 0.5% is a bonus, not a plan.
